The Bar Admissions application packet contains two sections, both of which should be carefully completed and submitted by the applicant:

• General Application (2 pages)
• Affidavit (2 pages)
• Authorization and Release form (1 page)

• NCBE Request for Preparation of a Character Report (go to www.ncbex.org/character.htm)
• Application (16 pages)
• Authorization and Release forms (3 pages)
• Blank Forms (Form Nos. 1 to 10)

Please answer each question in the forms in Sections One and Two. Type your answer or print using black ink. Sign all forms requiring your signature, including the Affidavit and each of the four Authorization and Release forms. Submit the original set and a copy, along with the appropriate fees, to the Bar Administrator's Office. The Bar Administrator will send Section Two to the National Conference of Bar Examiners.
